Surveys Child Form

The surveys child form lists the organization applying for the Accreditation Program.

Note: You cannot add a survey to the surveyor's profile from the surveys child form. To add a survey, go to the Accreditation Survey Area Profile>Surveyors Tab>surveyor information child form>Add button. For more information, see the Adding Surveyor Information help topic.

Surveys Child Form

You see the following columns on the Surveys child form:

Column Description
Survey Name Displays the name of the survey that this surveyor is involved in.

Surveys Grandchild Form

When you click the folder icon in the child form, you see the following columns in the grandchild form:

Column Description
Team Name Displays the name of the team this surveyor is a member of.
Team Members Lists the full names of the team members.
Team Role Lists the role for each team member.

Surveys Great Grandchild Form

When you click the folder icon in the grandchild form, you see the following columns in the great grandchild form:

Column Description
Evaluation Displays the title of the evaluation that this team member has submitted for this surveyor.

  more info icon to see more information about the a record on the child form.
  linked headings to sort the records.
  refresh the data in the form.
  export the data to a quick report for download.
